Requirements for Physical Therapy:

At Integrated Physical Therapy, when using insurance for coverage, we require the prescription of a physician prior to being seen on the first visit as well as every 30 days following.

Insurance for Physical Therapy:

Integrated Physical Therapy is a Participating Provider for Government Subsidized Medicare – treatments are billed directly to Medicare. We also accept all PPO insurance plans which have an out-of-network befefit. (See below for the list of insurances accepted.)

Fee for Service:

Patients may choose to pay for physical therapy and will be provided with a receipt for services, which can be submitted to insurance companies for reimbursement. You do have the option to ask Integrated Physical Therapy to submit your insurance bills for you.  There is no fee associated with this.  Please inquire at the time of your appointment.  All Physical Therapy at Integrated Physical Therapy is considered "out-of-network" for all insurance programs (except Medicare).

Why Fee For Service? This allows us to see patients for 60 minutes, one-on-one. It also allows us to charge a reasonable rate and provide care for those who chose not to use their insurance or have exhausted their insurance benefits but still require advanced care.

We believe that Physical Therapy and wellness is needed for everyone, even if you are not injured.  Our individualized programs will work with you to provide treatments that are designed to get YOU back to health.

Wellness: Massage Therapy and Personal Training

This is not covered by most insurance plans. We do not bill insurances for our Myofascial Release therapy as done under our Wellness Program. However, this is a more cost-effective way to receive Advanced Myofascial Release and specific exercises.
