Chad Crooks PTA
Physical Therapist Assistant
Chad was born and raised in Ohio. He served our country in the United States Air Force before attending and graduating in 1998 from Lourdes College in Toledo, Ohio. Chad relocated to south Florida in 2014 to continue his PT career.
He has practiced PT in the outpatient, inpatient and skilled nursing settings. Chad has special interest in treating orthopedic injuries as well as cervical and lumbar pathologies. He gets enjoyment and satisfaction assisting patients in returning to their prior level of function.
Chad enjoys attending sporting events and watching live music concerts. He also bikes to the beach and paddleboards the intercoastal waterways in his free time.
